[1] Take a new file of 300 pixels,300 pixels, of resolution 72 dpi in the RGB mode.
[2] Take the Rounded Rectangle Tool.
[3] Now create a new shape fill with Red color.
[4] Go to Blending Option and select Gradient Overly use these settings.
[5] Your image should be look like this.
[6] Now take the Pencil Tool use the following settings.
[7] Make the new layer then create a line see in the next.
[8] Now change the Opacity of line layer see in the next.
[9] Make the copy of 2nd layer.
[10] And now hold Cntl+Click on this layer to make the selection.
[11] Then fill the selection with black color.
[12] Create a new shape with the help of Rounded Rectangle Tool and fill with white color.
[13] Now go to Blending Option use the following settings.
[14] Your image should be look like this.
[15] Select the Layer-3 then right click to copy layer style.
[16] And then paste it layer-1 see in the next.
[17] Your image should be look like this.
[18] And now take the move tool and set your shape as shown image below.
[19] Now take the Text Tool and write your text.
[20] Make the copy of layer and change your text your final image should be look like this.
